Tourists in Cuba use mainly dollars. You can use pesos only for some food on the street, sometimes for transportation like trucks which go between cities and for local buses.
Try to have no more than 5$ in pesos. You can always exchange dollars in a very simple way: when paying the bill in your casa particular, ask to get the change in pesos. The rate never changes - 25 pesos.
About dollars: there are no many ATM machines in Cuba. All of them are in capitals of provinces. Another important thing - you can use only visa card in the ATM machines. Mastecard is not accepted.
You can withdraw money with all major credit card in banks. You need to go with your card and passport and withdraw dollars. Some banks also exchange TCs of Amex. It is possible in Havana, Santiago and maybe other places as well. Try to have 200$ when leaving for a new destination. In general, it should not be difficult to get dollars in every major tourist destination
